How to apply to be a 'Featured Therapist'

The first requirement is that you are on at least level 1 of our referral scheme. To make it as easy as possible to make your submission, we have provided a form which you can download to your computer. Once saved to your hard drive, you will be able to open up the file and enter your details, you then have the option to either email directly to us or print the form off so that it can be posted to the office.

Locate the document on your hard drive and open it up.

One at a time, delete the red texted fields and fill in your own details.

When completed, if you are posting your submission, just print it off. If you choose to email it, first save the completed document and then attach this saved file to your email.
